Background Advance Care Planning via Group Visits (ACP-GV) is a patient-centered intervention facilitated by a clinician using a group modality to promote healthcare decision-making among veterans. Participants in the group document a “Next Step” to use in planning for their future care needs. The next step may include documentation of preferences in an advance directive, discussing plans with family, or anything else to fulfill their ACP needs. This evaluation seeks to determine whether there are identifiable subgroups of group participants with (...) differing needs prior to delivery of the ACP-GV program and, if so, to use information about the subgroups to enhance the program offered to veterans in United States Department of Veterans Affairs (VA) healthcare settings. Methods We conducted a secondary analysis of national data from a quality improvement evaluation. Patient- and provider-level data from administrative healthcare records for VA users in all 50 states, territories, and the District of Columbia provides data on veterans attending ACP-GV during federal fiscal years 2018–2022 (N = 26,857). Latent class analysis seeks to identify the various subgroups of veterans based on their level of ACP self-efficacy before attending ACP-GV and any demographic differences across the resulting subgroups of veterans attending ACP-GV. ACP self-efficacy is derived from seven items obtained from a participant worksheet used during the group. Results Analysis revealed two distinct groups of veterans, distinguishable by their pre-ACP-GV levels of one aspect of ACP self-efficacy: prior knowledge of ACP. Veterans with higher prior knowledge of ACP are associated with an identified next step focused on checking their current AD status and updating it, and veterans with lower ACP prior knowledge are associated with identifying a next step to discuss ACP more fully with family. Differences in age, sex, race, ethnicity, and marital status exist across subgroups of veterans. Conclusion Greater attention must be paid to ACP and veterans’ prior knowledge of ACP to consistently encourage annual review and status updates. (shrink)

A major challenge in neuroscience is to understand what happens to a brain as it ages. Such insights could make it possible to distinguish between individuals who will undergo typical aging and those at risk for neurodegenerative disease. Over the last quarter century, thousands of human brain imaging studies have probed the neural basis of age-related cognitive decline. “Aging” studies generally enroll adults over the age of 65, a historical precedent rooted in the average retirement age of U.S. wage-earners. A (...) consequence of this research tradition is that it overlooks one of the most significant neuroendocrine changes in a woman’s life: the transition to menopause. The menopausal transition is marked by an overall decline in ovarian sex steroid production – up to 90% in the case of estradiol – a dramatic endocrine change that impacts multiple biological systems, including the brain. Despite sex differences in the risk for dementia, the influence that biological sex and sex hormones have on the aging brain is historically understudied, leaving a critical gap in our understanding of the aging process. In this Perspective, we highlight the influence that endocrine factors have on the aging brain. We devote particular attention to the neural and cognitive changes that unfold in the middle decade of life, as a function of reproductive aging. Next, we consider emerging evidence from animal and human studies that other endocrine factors occurring earlier in life (e.g. pregnancy, hormonal birth control use) also shape the aging process. The inconsistent definition of empathy has had a negative impact on both research and practice. The aim of this article is to review and critically appraise a range of definitions of empathy and, through considered analysis, to develop a new conceptualisation. From the examination of 43 discrete definitions, 8 themes relating to the nature of empathy emerged: “distinguishing empathy from other concepts”; “cognitive or affective?”; “congruent or incongruent?”; “subject to other stimuli?”; “self/other distinction or merging?”; “trait or state influences?”; “has (...) a behavioural outcome?”; and “automatic or controlled?” The relevance and validity of each theme is assessed and a new conceptualisation of empathy is offered. Ecological restoration and native landscaping are increasing, particularly in the American Midwest, where they form part of the area's history and culture of conservation. But practitioners rarely pause to ask philosophical questions related to categories of native and invasive or human control and harmony with nature. This article brings philosophy into conversation with practice, using members of Wild Ones Native Landscaping, a non-profit headquartered in Neenah, WI, as a case study. Data documenting poor understanding among research participants and real-time efforts to assess comprehension in large-scale studies are focusing new attention on informed consent comprehension. Within the context of biobanking consent, we previously convened a multidisciplinary panel to reach consensus about what information must be understood for a prospective participant’s consent to be considered valid. Subsequently, we presented them with data from another study showing that many U.S. adults would fail to comprehend the information the panel had deemed essential. When asked (...) to evaluate the importance of the information again, panelists’ opinions shifted dramatically in the direction of requiring that less information be understood. Follow-up interviews indicated significant uncertainty about defining a threshold of understanding and what should happen when prospective participants are unable to grasp key information. Aristotle's Topics is a handbook for dialectic, which can be understood as a philosophical debate between a questioner and a respondent. In book 2, Aristotle mainly develops strategies for making deductions about 'accidents', which are properties that might or might not belong to a subject (for instance, Socrates has five fingers, but might have had six), and about properties that simply belong to a subject without further specification. In the present commentary, here translated into English for the first time, Alexander (...) develops a careful study of Aristotle's text. He preserves objections and replies from other philosophers whose work is now lost, such as the Stoics. He also offers an invaluable picture of the tradition of Aristotelian logic down to his time, including innovative attempts to unify Aristotle's guidance for dialectic with his general theory of deductive argument (the syllogism), found in the Analytics. The work will be of interest not only for its perspective on ancient logic, rhetoric, and debate, but also for its continuing influence on argument in the Middle Ages and later. (shrink)

I examine the dynamics of measure development using two case studies: temperature, and health-related quality of life. I argue, following Bas van Fraassen and Leah McClimans that in each case these dynamics have a hermeneutic structure. Measure development is plagued by epistemic circularity, as is the task of interpreting a text, and similar strategies can be used in both measure development and hermeneutics to overcome that circularity. I show that Hans Georg Gadamer’s philosophical hermeneutics in particular are an effective lens (...) through which to examine the development of the temperature standard as described by Hasok Chang. Despite similar grounding in hermeneutics, I note an important difference between measure development for temperature and for health-related quality of life. Namely, while the meaning of temperature can be standardized, the meaning of health-related quality of life cannot. This standardization of meaning for the temperature concept represents a limit to the analogy with hermeneutics. Finally, I argue that the indeterminacy we find in health-related quality of life measurement is a result not only of analogy with the hermeneutic task, but of full-fledged participation in it. By my count, there are forty-six (...) books to his name, including these two from 2013. And yet in the preface to Without Apology, he writes, “I am often asked, ‘How many books have you written?’ My truthful answer must be, ‘I have no idea.’ … I could count them, but I have no interest in doing so” (ix). Hauerwas’s reluctance to number his career in books shows his preference for smaller units of measurement (“how some reader may find this sermon, or that sentence or chapter … helpful”), but it also reflects his view that he is not the sole author of his works. To his credit, he mentions two influential women—his wife, Paula Gilbert, an experienced clergywoman, and his assistant, Carole Baker, who gives the sermons a “work-over” after they are written (ix).Those who know Hauerwas know what to expect from these volumes: thoughtful arguments that unflinchingly assert the centrality of Christ and of the Church; a heavy dose of pacifism; major appearances from John Howard Yoder and Karl Barth (as well as multiple cameos from fascinating authors who are now on my to-read list); and accessible, open prose outlining clear, thought-provoking arguments. The two books reviewed here are quite different from one another: Without Apology is a book of sermons; Approaching the End is a book of essays loosely organized around a theme of eschatology. But both contain the hallmarks of “classic Hauerwas”—his signature mix of virtue ethics, communitarian interests, and narrative sensibilities.Without Apology contains recent sermons, primarily in Episcopal churches. Drawing on lectionary readings, Hauerwas brings his own theological interpretation and creates fairly short, satisfying sermons, which he readily describes (in Paula Gilbert’s words) as “tight” (87). This collection also includes four sermons on the topic of priesthood and some other writings of note: the already-classic “An Open Letter to Christians Beginning College” (good for use with Christian undergraduates) and “Sexing the Ministry” (food for thought for [End Page 215] seminarian and clergy alike). Without Apology manifests several major themes, but the biggest of these connects with the title: Hauerwas is “unapologetic” about his Christian faith and doesn’t engage in theology of the “apologetic” style. His emphasis, rather, is on being truthfully present with one another in the church community.Approaching the End promises eschatological reflections and delivers on the promise. Although he is writing about eschatology, Hauerwas strongly maintains that he is not reaching his own “end” and disavows any contention that Approaching the End is a kind of “swan song” (xiv). He addresses politics and war in several chapters. In “The End of Sacrifice: An Apocalyptic Politics,” he makes the case that if Christians believe Jesus’s sacrifice was “the end of all sacrifices,” then we should not “continue to participate in the sacrifice of war” (36). Similar themes emerge in “War and Peace,” later in the volume. In “Church Matters: On Faith and Politics,” Hauerwas writes, “One of the challenges Christians confront is how the politics we helped create has made it difficult to sustain the material practices constitutive of an ecclesial culture to produce Christians” (68). He welcomes the “end” of churches and denominations as we know them because “it means that the church is finally free to be a politics” (86).Hauerwas also discusses the future of churches and denominations. “The End of Protestantism” reiterates that “if the church is to be faithful … then we must recover what it means for the church to be an alternative politics” (92). Seeking justice, as Christians, means seriously reconsidering our food consumption in light of multiple instances of injustice: maltreatment of workers, animals, and the environment; and misdistribution of food both globally and domestically. A variety of solutions—including boycotts, labeling, local consumption, generous donations, and Food Sovereignty—would lead to a more just food system.

A worldwidemental health crisis is expected, as millions worldwide fear death and disease while being forced into repeated isolation. Thus, there is a need for new proactive approaches to improve mental resilience and prevent mental health conditions. Since the 1990s, art has emerged as an alternative mental health therapy in the United States and Europe, becoming part of the social care agenda. This article focuses on how visual esthetic experiences can create similar patterns of neuronal activity as those observed when (...) the reward system is activated. The activation of the reward structures could have a stress buffering effect, given the interdependence observed between the reward and stress systems. Therefore, could visual esthetic experiences stimulate mental resilience? And if this were the case, could art-based interventions be offered for mental health in the context of COVID-19 and beyond? As library collections and services have increasingly moved from print to digital, much of the work that used to be done by libraries themselves with regard to creating, maintaining, and managing the systems that hold collections and facilitate user access to them is now done primarily by vendors. This change to the information services landscape for academic libraries is the occasion not only of technical and procedural challenges, but also some internal conflicts concerning the ethical demands of the library profession. This essay examines the ways in which these conflicts become acute and obvious when attention is paid to the way in which attempts to promote equity, diversity, and inclusion (EDI) are undermined by the handover of library services to vendors, even when—or perhaps especially when—those vendors use their control of content and platforms to commodify diverse content instead of making the necessary systemic changes required by truly effective EDI efforts. Public streets are central to the built environment, where individuals seek a fair share of the roadway’s benefits and harms. But the American street, an asphalt landscape typically defined and designed for cars, can be inaccessible, unhealthy, and dangerous for the non-motorized, whose transportation choices have the smallest ecological footprint. Concern for social equity and sustainability requires rethinking the street geographically and ethically, and asking: “In what sense is the street a space of justice? How do traditional street regulation and (...) design manifest ethical priorities? And what might a more just street look like, in theory and practice?” Such questions prompt one to engage both the spatial and moral, thus drawing from critical geography and ethics to analyze roadways in terms of fairness and relational wholeness, and argue for what might be called a shalom street. Engaging such ethical concepts with the technical vocabularies of street regulation and design requires analyzing how national model standards and their interpretation enforce and materialize justice on the street.The promise of more just alternatives such as more sustainable and fair “Complete Streets” to live in needs to be explored. We present a novel way of accounting for similarity judgments. Our approach posits that similarity stems from three main sources—familiarity, priming, and inherent perceptual likeness. Here, we explore each of these constructs and demonstrate their individual and combined effectiveness in explaining similarity judgments. Using these three measures, our account of similarity explains ratings of simple, color-based perceptual stimuli that display asymmetry effects, as well as more complicated perceptual stimuli with structural properties; more traditional approaches to similarity solve one or the (...) other and have difficulty accounting for both. Overall, our work demonstrates the importance of each of these components of similarity in explaining similarity judgments, both individually and together, and suggests important implications for other similarity approaches. The word yoginī is an ambivalent term, generally defined as a female yogin. For the purposes of the University of Hawai′i′s Center for South Asian Studies’ Symposium on the Ineffable in Religion and Ritual, I envisioned the ambivalence of the yoginī as characterized by semantic ineffability. This ineffability is seen in the divergence of definitions and descriptions of the yoginī in text and ethnography. The tantras portray her as flying, blood thirsty, and the object of tantric sex rites; she is (...) rarely portrayed as a yoga practitioner. Human yoginīs in India, by in contrast, often identify themselves as sannyāsinī (renunciate), practice some form of yoga, and are often celibate. To address the question of why textual and human yoginīs bear little in common, I render a polythetic definition of the yoginī by first looking at the many modes of definition and classification used to render meaning of the concepts that are integral to the formation of the yoginī. I then problematize the categorical construction of the yoginī and the disjunction between textual references to the yoginī in tantras of the Vidyāpīṭha (“Wisdom Mantra Corpus” or “Seat of Wisdom”), the Śaiva and Śākta Purāṇas, and ethnographic accounts of embodied yoginīs. These three sources, which emerge from various sectarian, historical, and social milieus offer interrelated yet distinct descriptions of the yoginī and as such provide a broad base of signification upon which to render a polythetic definition. We describe a new approach for developing and validating cognitive process models. We develop graphical models (specifically, hidden Markov models) both from human empirical data on a task, as well as from synthetic data traces generated by a cognitive process model of human behavior on the task. We show that considering differences between the two graphical models can unveil substantive and nuanced imperfections of cognitive process models that can then be addressed to increase their fidelity to empirical data.

This commentary examines twelve stories in which parents recount how they (and often their co-parent) decided whether or not to circumcise their newborn sons. Several debated whether this should be their decision to make. The stories offer an intimate glimpse into people's efforts to do the best for children in a context of incomplete and changing information and intense public controversy. The commentary explores the diverse meanings and contradictory commonsense beliefs that surround foreskin removal in the United States today. Considering (...) these parents' reflections—and their silences—can help us appreciate the real-life consequences of debates about the ethics of male circumcision. (shrink)

In qualitative interviews with a diverse group of experts, the vast majority believed unregulated researchers should seek out independent oversight. Reasons included the need for objectivity, protecting app users from research risks, and consistency in standards for the ethical conduct of research. Concerns included burdening minimal risk research and limitations in current systems of oversight. Literature and analysis supports the use of IRBs even when not required by regulations, and the need for evidence-based improvements in IRB processes.

They often rely on a conceptual tool called the quality-adjusted life year that discounts the value of years lived in a state of disability relative to years lived in full health. Can humans flourish without destroying the earth? In this book, experts on many of the world's major and minor religious traditions address the question of human and earth flourishing. Each chapter considers specific religious ideas and specific environmental harms. Chapters are paired and the authors work in dialogue with one another. Taken together, the chapters reveal that the question of flourishing is deceptively simple. Most would agree that humans should flourish without destroying the earth. But not all humans have equal (...) opportunities to flourish. Additionally, on a basic physical level any human flourishing must, of necessity, cause some harm. These considerations of the price and distribution of flourishing raise unique questions about the status of humans and nature. This book represents a step toward reconciliation: that people and their ecosystems may live in peace, that people from different religious worldviews may engage in productive dialogue; in short, that all may flourish. The vice of acedia deserves—and rewards—a closer reading than is implied in the old rendering ‘sloth’, or even in contemporary readings of ‘spiritual sloth’. Such is at least true in Aquinas’s Summa Theologiae, the subject of the following close reading of this enigmatic vice. Investigating the question on acedia and its grounding in portions of I-II, I first establish acedia’s basis not in a sovereign spiritual ‘choice’ but in the sensitive appetite and the passion of sorrow. This turns the portrait (...) of acedia’s ‘patients’ from those who avoid to those who embrace the labors of love, those who experience acedia’s sorrow in the very midst of pursuing divine joy. A third section steps back to show the entire problem of acedia, including its ‘slothful’ effects, in terms of this one fundamental sorrow that suppresses joy—real rest—in the Lord. I end with Thomas’s remedy, the hope-charged possibility of rising up in defense of the divine joy, turning back acedia’s sorrow at every point of its attack and reclaiming the pleasure of sharing in God’s life. GraphicalThe need to encourage individuals as active change agents for sustainability transitions has led researchers across disciplines to conceptualize over 70 constructs to assess relevant dispositions to environmental protection and green consumption behaviors. The generated knowledge is, however, fragmented by an unconsolidated set of constructs developed within parallel literature streams. We, hence, use an integrative review method to capture conceptual and operational similarities and distinctiveness of constructs across disciplines in the literature, attempting to unify the knowledge base. Thereby, we identify (...) 34 conceptually distinct constructs relevant for the thematic synthesis on individual-level constructs framing contributions to environmental measures and issues. We followingly propose a taxonomy, systemizing constructs based on their concept type and contextual scope of the environmental challenge addressed. We capture these dimensions in critically assessing relevant and salient conceptual and operational features. We thus create a consolidated picture of extant constructs capturing individual-level environmental sustainability by which we intend a three-fold contribution to the interdisciplinary field. First, the taxonomy and guiding framework for the choice of constructs should assist substantive researchers in identifying appropriate constructs of interest. Second, the systematic integration of similar concepts available in parallel literature streams should assist future endeavors aiming at integrating substantive findings with regard to antecedents, consequences, and other relevant variables. Finally, the taxonomy revealed that conceptualizations mainly scatter around specific combinations of types and scopes while others remain unaddressed. Based on our critical assessment of the relevant features and resulting taxonomy, we identify avenues for future research dedicated to enhancing conceptual rigor and measurement quality in the field and introducing concepts addressing missing but potentially valuable combinations of types and scopes. We conclude that researchers engaging in the proposed avenues with conceptual, methodological, or empirical contributions should consider four critical aspects to advance knowledge accumulation and prevent fragmentation in the interdisciplinary field. We thereby hope to pave the way for a collective, interdisciplinary knowledge base of concepts used to describe and assess individual’s pro-environmental dispositions and practices of green consumption.
